What is the Hague Complaint Registry?
The Hague Complaint Registry is a service for American citizens provided by the U.S. Department of State. This website allows you to file a complaint regarding your adoption process. The Hague Convention on Intercountry Adoptions Complaint Registry will forward your complaint to the appropriate Accrediting Entity for action, and the Department will monitor complaints about accredited agencies or approved persons after receiving information from you.
To submit a complaint, simply click on the link to the Complaint Form, fill out the complaint fields as they pertain to your case, carefully review your information and click submit when finished. If you wish, you may also print out a form and fax it to the U.S. Central Authority at 202-736-9080 or mail it and any supporting documents to:

To Submit a Complaint you must perform the following actions:
Click on the Complaint form Link to Open Step 1 of the Complaint Form
Enter Your Complaint Category from Step 1
Enter Your Contact Information in Step 2 and Described the Complaint in Step 3. (The fields with the asterisk, *, beside them are required fields)
Click the Submit button
The system will generate a Confirmation Number. Please keep this number to reference this complaint in the future.
Review the complaint and click the Confirm and Submit button at the bottom of the form.

What is the Purpose of the Confirmation Number?
The Confirmation Number is generated once you have submitted a valid complaint. It is used as a reference point for any future questions you may have regarding the status of your complaint. Therefore, it is recommended that you keep this number for your records.

What is the Purpose of the Privacy Act?
The provisions of the Privacy Act are designed to protect the privacy rights of Americans. As a general rule, officers may not reveal information regarding an individual American’s location, welfare, intentions, or problems to anyone, including family members and Congressional representatives, without the express written consent of that individual. This consent is called a Privacy Act Waiver. Under certain limited conditions, such as incapacity of the American, limited information can be released to family members without a Privacy Act Waiver.
